Vivante offers a groundbreaking partnership with Hoag to provide comprehensive care to residents in the comfort of their homes
By partnering with Hoag
we are combining Hoag’s world class health care system with Vivante’s commitment to the best in senior living.
The partnership ensures a wide-ranging scope of medical and wellness services for residents provided by a full-time team of doctors and therapists so that residents can remain in place for critical rehabilitation and medical treatment.
The collaboration also ensures a full-time staff is there to provide a proactive approach to senior care that includes on-site doctor visits, physical and occupational therapy, neurological evaluation and assessment of dementia, and other medical services including clinical trials for cancer screening.
Primary care m.d. on site
Convenience for residents that prefer to be treated at home with a personalized and comprehensive care plan. Also, creating a collaborative approach with residents, families, and specialist physicians.
Physical & occupational therapy
As an effort to keep your loved one out of skilled nursing, both Medicare Part A and B care are available. Rehab in the comfort of home, or in our newly renovated space, “VIVAH.” (Vivante Initiative for Vital Aging with Hoag).
Hospice care
Offering coordination of medications and equipment for comfort and symptom management. Psychological and spiritual support for patients, families, and caregivers.
grief & emotional support
Bereavement support for family and loved ones through illness. Support for residents who struggle to adjust to assisted living or memory care by trained professionals.
clinical trials
Hoag Hospital is always forging new paths to excellent care. Vivante residents will have the opportunity to participate in new studies to help further the hospital’s research and benefit the resident at no cost.
neuroscience programming
We are working with doctors at the Neurosciences Institute to provide the most up-to-date cognitive care and programming to our residents. This includes virtual reality for therapeutic purposes and the Montessori Method for dementia.
cancer awareness & care
Provide skin cancer awareness and checks for residents in our community through doctors from the Patty and George Hoag Cancer Center.
diabetes screenings
Educating residents on the importance of diabetes prevention through educational lectures and 1:1 consultations with our Hoag physicians in-house.
